He was a few years out from his prostatectomy. Wise man. Smart wife. They'd been to prominent universities and seen prominent urologists.<\/p>\r\n<p>And neither of them had ever heard the phrase <strong>\"penile rehabilitation.\"<\/strong><\/p>\r\n<p>It got worse. Somewhere along the way, someone had given him a penis pump \u2014 with no pressure gauge. Someone had also given him what was called a \"P-Shot.\" From the way his wife described it, the plasma went in some odd location. My bet is that the person who did it only <em>said<\/em> they were part of our group.<\/p>\r\n<p>Here's what bothered me most: <span class=\"ps-red\">a man who wanted his love life back had spent real money on half-measures, and nobody had given him the whole protocol.<\/span><\/p>\r\n<p>If someone in our world doesn't know something, shame on me for not teaching it. So this page is me getting back on my soapbox \u2014 and inviting you to learn to do this the right way.<\/p>\r\n<h2>What's the Problem?<\/h2>\r\n<p>You already know the numbers on erectile dysfunction. What you may not see is what the men don't say out loud.<\/p>\r\n<p>They don't come in asking for help with their erections. They come in for their blood pressure, their knee, their \"annual.\" The ones who <em>do<\/em> bring it up have usually been pushed in the door by their wives. (Yes \u2014 the wives bring their husbands. That's why gynecologists are some of our best P-Shot\u00ae providers.)<\/p>\r\n<p>And when a man finally does sit down in front of you, here's the hard truth: <strong>he is not an easy case.<\/strong> If he were, the first doctor would have fixed him.<\/p>\r\n<p>A man who is willing to pay cash has usually already seen one or two physicians who take his insurance. He likes them. He's a little embarrassed to be seeing you at all \u2014 almost like he's cheating on his doctor.<\/p>\r\n<p>So you are being handed the hardest cases. And most of us were trained with only two moves: write a prescription, or refer him on.<\/p>\r\n<h2>What's Been Tried?<\/h2>\r\n<p>Let's be fair to every tool, because I use most of them.<\/p>\r\n<ul>\r\n<li><strong>The pills work.<\/strong> Daily tadalafil and its cousins raise the IIEF-5 (SHIM) score by roughly seven points in the studies. Good. But they don't rebuild tissue, and some men don't respond at all.<\/li>\r\n<li><strong>The pump works<\/strong> \u2014 when it's used correctly. But I've had at least twenty men tell me, \"I already have a pump.\" When I ask whether it has a pressure gauge, the answer is often no. Then I put a real one on, and they're shocked at what they'd been missing.<\/li>\r\n<li><strong>Walking works.<\/strong> One study of tennis three times a week bumped the score by about seven. (At my level, tennis is mostly watching the ball whiz by \u2014 so it doesn't help much.)<\/li>\r\n<li><strong>Injections and implants work<\/strong> \u2014 and most men dread them, but for some, they are a miracle, life changer. Most men just want that as a last resort.<\/li>\r\n<li><strong>\"Generic PRP\" sometimes doesn't work.<\/strong> The one placebo-controlled study that failed used half the dose and did not include the activation process or the injection methods we use. In my opinion, the methodology was poor. JAMA has made the point that, with these therapies, <a href=\"https:\/\/doi.org\/10.1001\/jamanetworkopen.2022.45075\" target=\"_blank\" rel=\"noopener\">the method matters<\/a>.<\/li>\r\n<\/ul>\r\n<p>And then there's the option most often chosen: <strong>wait.<\/strong> There is still no standard protocol for penile rehabilitation in the textbooks. The debate began around 2010 and hasn't ended.<\/p>\r\n<p>But imagine you're the man \u2014 or his wife. <span class=\"ps-red\">Do you really want to wait another ten or twenty years for the research to settle the question?<\/span> Or do you want the best research we have now, combined with the best understanding of the physiology and physics of an erection?<\/p>\r\n<h2>What's New?<\/h2>\r\n<h3>The Priapus Shot\u00ae procedure<\/h3>\r\n<p>In 2010, I designed a very specific way to isolate a patient's own platelet-rich plasma (and other blood-derived growth factors) and inject it in a very specific way to improve the health and function of the penis. The outside research has kept stacking up:<\/p>\r\n<ul>\r\n<li>A <a href=\"https:\/\/doi.org\/10.1016\/j.jsxm.2021.03.008\" target=\"_blank\" rel=\"noopener\">double-blind, randomized, placebo-controlled trial<\/a> showing PRP improves erectile function (Poulios et al., <em>Journal of Sexual Medicine<\/em>, 2021).<\/li>\r\n<li>A <a href=\"https:\/\/doi.org\/10.1093\/jsxmed\/qdae131\" target=\"_blank\" rel=\"noopener\">2024 systematic review and Bayesian network meta-analysis<\/a> of regenerative therapies for ED (Hinojosa-Gonzalez et al., <em>Journal of Sexual Medicine<\/em>).<\/li>\r\n<li>A <a href=\"https:\/\/doi.org\/10.4081\/aiua.2024.12687\" target=\"_blank\" rel=\"noopener\">2024 GRADE-assessed meta-analysis of randomized trials<\/a> of intracavernosal PRP for mild-to-moderate ED (Yogiswara et al.).<\/li>\r\n<li>A <a href=\"https:\/\/doi.org\/10.1080\/13685538.2025.2472786\" target=\"_blank\" rel=\"noopener\">2025 meta-analysis<\/a> of PRP alone or combined with low-intensity shockwave (Zhou et al., <em>The Aging Male<\/em>).<\/li>\r\n<\/ul>\r\n<figure class=\"ps-figure\"><a href=\"https:\/\/priapusshot.com\/research\/\" target=\"_blank\" rel=\"noopener\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/priapusshot.com\/members\/wp-content\/uploads\/2021\/06\/IMG_6731-776x1024.jpeg\" alt=\"The Journal of Sexual Medicine, May 2021 cover: The Effect of PRP versus Placebo on Erectile Function\" width=\"776\" height=\"1024\" \/><\/a>\r\n<figcaption>The May 2021 cover of <em>The Journal of Sexual Medicine<\/em>: the effect of PRP versus placebo on erectile function. <a href=\"https:\/\/priapusshot.com\/research\/\" target=\"_blank\" rel=\"noopener\">See all the research \u2192<\/a><\/figcaption>\r\n<\/figure>\r\n<p>Does it work every time? Of course not. Does it have a statistically significant success rate? <strong>Yes.<\/strong> And much more research needs to be done \u2014 which is exactly why I want more good clinicians doing it the standardized way and reporting what they see.<\/p>\r\n<h3>The Priapus Toxin\u2122 procedure<\/h3>\r\n<p>This is the one most providers haven't heard of, and it's where many of the hardest cases turn around.<\/p>\r\n<p>Botulinum toxin injected into the corpus cavernosum has now been studied in double-blind trials using <a href=\"https:\/\/doi.org\/10.1111\/andr.13010\" target=\"_blank\" rel=\"noopener\">Botox\u00ae<\/a>, <a href=\"https:\/\/doi.org\/10.3390\/toxins11050283\" target=\"_blank\" rel=\"noopener\">Dysport\u00ae<\/a>, and <a href=\"https:\/\/doi.org\/10.3390\/toxins14040286\" target=\"_blank\" rel=\"noopener\">Xeomin\u00ae<\/a>. And here's the part that should make you sit up: <span class=\"ps-red\">those studies were done in men for whom the pills did nothing<\/span> \u2014 and still showed roughly a <strong>40% success rate<\/strong>. With Xeomin, 100 units lasted about nine months.<\/p>\r\n<p>Forty percent of men had already failed everything else. Knowing that and <em>not<\/em> offering it means you're not offering the best of what we know. In the course, I'll also show you the P-Shot 100\u2122\u2014combining the two in a single visit.<\/p>\r\n<h3>The whole protocol, not just a shot<\/h3>\r\n<p>Here's what I want you to understand: the procedure is the centerpiece of penile rehabilitation, but the results come from the <strong>stack.<\/strong> You'll learn my full protocol \u2014 including the one I use for men recovering from prostate surgery (only parts of this protocol may be needed for someone with the beginnings of ED without prostate problems):<\/p>\r\n<ol>\r\n<li>Call the urologist \u2014 while the patient is in your office.
