Alexandra Runnels, MD

Dr. Alexandra Runnels is a board-certified OB/GYN who has spent nearly three decades studying, treating, teaching, and researching what most of medicine was content to ignore: women's sexual function.
She founded a concierge regenerative and aesthetic gynecology practice specializing in sexual medicine, genitoplastic surgery, and facial aesthetics, and began using platelet-rich plasma clinically for over a decade—long before it was fashionable.
She helped her husband develop Clitoxin®, a method of delivery and use of botulinum toxin for female sexual dysfunction, and she was its first subject. That same instinct for precision over palliation led to a new method of treating bladder dysfunction through a single clitoral delivery route.
An international speaker, published researcher, and author, Dr. Runnels most recently presented new Clitoxin data at the ESAG/BSARG World Congress in Bucharest in October 2026. She teaches the Clitoxin method through the Cellular Medicine Association, with Brazil next on the calendar.
Her approach is a hybrid of regenerative therapies, conventional medicine, restorative surgery, and home protocols — tailored to each patient and applied to the conditions others call difficult: vulvar lichen sclerosus, vulvodynia, clitorodynia, interstitial cystitis, overactive bladder, vaginismus, dyspareunia, clitoral phimosis, and anal fissure.
Her position is simple. A woman's sexual function is hers by birthright — to be celebrated, explored, studied, and, when broken, fixed.
She currently practices in Fairhope, AL and shares an office with her husband, Dr. Charles Runels.
